Before filling out the forms online, please read the system requirements carefully. If you do not have the listed system requirements, you may not be able to complete the form online.
DS-156 Form: Must be completed by all non-immigrant visa applicants. For persons under the age of 16, the form may be completed and signed by one of the parents or a legal guardian. Instructions for completing Form DS-156 are available in Russian and Romanian.
DS-157 Form: Must be completed in English by all male non-immigrant visa applicants between the ages of 16-45. Also required for all applicants over age 16, male or female, who are citizens of, or born in Cuba, Iran, Libya, North Korea, Sudan, or Syria.
DS-158 Form: Must be completed in English by all F, J, and M category visa applicants, including spouses and children applying for F2, J2, and M2 visas.

Barcodes that cannot be read by our scanner cannot be accepted.
