The following form (DS-156) needs to be completed by all non-immigrant visa applicants. For persons under the age of 16, the form may be completed and signed by one of the parents or a legal guardian:

The following supplemental form (DS-157) needs to be completed by all male non-immigrant visa applicants between the ages of 16-45, regardless of nationality:

The following supplemental form (DS-158) needs to be completed by all F, J, and M category visa applicants, including spouses and children applying for F2, J2, and M2 visas, regardless of nationality.
